If you do decide to play on Medium, it's also not so bad, you only seem to lose the Ambient Occlusion effect that you can see on the red banner things on the left. It's almost like the builders on Very High are just more generous with their materials when they built Kirkwall. Didn't care for that texture bump you can see on the floor, and the fatter pillar on the right.

While the difference between Very High and High was not perceivable enough when I actually played it (I preferred High because I could maintain 60 fps and above when I played on High). Personally I played on High, because there doesn't seem to be any difference between Maxed and Very High. I think I used a high resolution texture mod made by a modder outside of Bioware. The good news is, the graphical differences from maxed settings to medium don't seem to be that bad. I have an i5-2500k and Nvidia 760 GT and when I run it on max I get 40 to 60 fps.
